AFAIK, a feature of the 40 position Multifix is that the 3 holes are not spaced at 120 deg. but differ by 2.25 deg., ie 1/4 x 9 deg.
I have no idea if the specifics of what you say is correct, but what you describe might have solved a Multifix mystery.
It has been remarked here before that with 9 degee indexing, the toolpost cannot do common angles like 30, 60 or 15 degrees.
With judicious angular spacing of the holes, that limitation could be overcome. Not pondering this for long, so this could be wrong, but if the holes had something like (n x 9) +/- 3 degrees between them, I think that would fill in the blanks (15 is 18-3; 30 is 27+3; 60 is 63-3).
