I would have thought that as a basic design idea, you’d have an enclosed worm system as a replacement for the original quill feed handles, and move these out by the depth of your additional section with the worm drive, etc. and have a means of reattaching the originals. The only real snag with any design like this is that you have to have a means of detaching the worm drive so that the original handles can still be used. On our Warco Major mill/drill this is achieved by a friction lock that when loosened, releases the worm gear.
The only other snag with the whole arrangement is that as Neil points out, bench drills are generally pretty crude in their quill arrangements anyway, and to make any serious use of an attachment like this (which I think is a good idea), you may well have to make arrangements to tighten up the workings of the whole quill to get all of the potential accuracy benefits.
It’s of interest to me, because I have a Meddings Pacera to rebuild, and adding a fine feed is something I’ve contemplated.
